Grading of instability 0 No demonstrable abnormal motion by comparison with the opposite knee
1+ Demonstrable, but not disabling, abnormal motion by comparison with the opposite knee; abnormal motion usually not appreciated by the patient
2+ Definite, somewhat disabling, abnormal motion by comparison with the opposite knee; abnormal motion recognized by the patient, the degree of disability varying with the condition of the other structures normally responsible for knee stability
3+ Obvious, moderately disabling, abnormal motion quite apparent to the patient; the degree of disability varying with the condition of the other structures normally responsible for knee stability
4+ Gross, very disabling, instability with no demonstrable evidence of any ligament-capsular check-rein effect limiting abnormal motion during testing. The knee gives away and support is required during ordinary activities
Grading of knee disability none Patient engages in all activities without restriction
Grade 1 Patient participates with reduced proficiency in organized athletics but should not play football without bracing. He is not conscious of instability during his regular activities and has no pain, swelling or restriction of motion. Instability is 2+ or less
Grade 2 Patient is unable to engage in vigorous sports including football, baseball, basketball and tennis, but can play golf or volleyball and hike. He has episodes of discomfort or swelling, or his knee lacks 20° of flexion. Instability is between 2+ and 3+
Grade 3 Patient is unable to participate effectively in any sports and has some trouble in ordinary walking, climbing stairs and squatting. He has pain and swelling as well as loss of extension, loss of more than 20° of flexion, or both. Instability is 3+
Grade 4 Patient is severely restricted in all activities including walking, climbing stairs and the like, often requiring a brace, cane or crutches. Instability is more than 3+
